There is no charge to send your records to another physician.  If you want to pick up your entire medical file and it is extensive, we may have to send it out to be copied, and there will be a charge.  If your file is smaller and we can copy it in-house, or if you just need certain documents, we will not charge.  We do charge to send copies of records to attorneys.

The Authorization for Release of Medical Information should be filled out and returned to our office by mail, in person or faxed to Medical Records at 336-275-0433.
