Please call us at least 3 days in advance to request medical records; depending on the size and scope of the records requested, it could take up to 7 days.  If you are picking up your records, you will need to sign a release.  If your doctor requests that we send records, you do not need to sign a release.

The Authorization for Release of Medical Information should be filled out and returned to our office by mail, in person or faxed to Medical Records at 336-275-0433.
