Do I have to sign a release for you to give my medical information to another physician?
 
 

The Health Insurance Portability and Accountability Act (HIPAA) allows us to share your medical information with another physician for treatment purposes, so you would not need to sign a release to have us send your information to another doctor.  For more information about how and when we may disclose your medical information, see our Patient Privacy page.

For situations where a release is required, the Authorization for Release of Medical Information should be filled out and returned to our office by mail, in person or faxed to Medical Records at 336-275-0433.
